Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region. Our team at Farleigh has grown and evolved to meet the changing needs of the community we serve, and we are looking for highly motivated Registered Nurse Plus/HCP Plus to join our Community Teams.
Are you a motivated, forward thinking, talented individual interested in delivering excellent specialist palliative care within a multidisciplinary team to patients and their families within mid Essex?
This exciting Registered Nurse Plus/HCP Plus post will provide an opportunity for individuals with excellent communication skills, clinical knowledge and leadership skills to act as team role models and be part of developing new ways of working, alongside quality improvement. This will include caseload management and undertaking assessments including MCA. Alongside MDT colleagues, providing excellent symptom control, psycho-social and spiritual support for patients who are under the care of the community teams supporting patients and their significant others through to end of life care.

In addition, for RN +/HCP roles:
- Access to an internal advanced symptom control course
- Train the trainer for developing teaching skills
