This section provides information and resources for health and social care professionals supporting patients with specialist palliative and end of life care needs. Here you can find guidance on making clinical referrals, along with updates on education and training.

Referrals

We accept referrals for patients with specialist palliative and end of life care needs. Please ensure referral criteria are met before submitting a clinical referral.

Who can be referred?

We support people who:

  • Have advanced, progressive or incurable conditions
  • Are experiencing general frailty with rapid deterioration or end stage illness
  • Require rapid access to end of life care, including personal care, in the last three months of life
  • Have existing conditions at risk of sudden acute crisis
  • Have life-threatening acute conditions caused by sudden catastrophic events

Eligibility

Patients referred to Farleigh Hospice should:

Age requirement

Be aged 18 or over (exceptionally, 16–18 year olds may be considered following clinical agreement)

Local connection

Live in mid Essex and/or be registered with a mid Essex GP

Consent and referral

Provide consent for referral to Farleigh Hospice services. Referrals from outside the area may be considered in certain circumstances.

What referrals may include

  • Difficulties managing pain and other symptoms
  • Complex or escalating care needs
  • Support with palliative rehabilitation
  • Complex emotional or holistic care needs

After referral

All referrals are reviewed and triaged by our clinical team.
The appropriate Farleigh Hospice service will then contact the patient or referrer. For admission to our Inpatient Unit, a separate referral process applies. Farleigh Hospice does not provide long-term care beds.
