Referral criteria

Patients may be suitable if they have:

• Advanced, progressive or incurable illness
• General frailty with rapid deterioration
• End of life care needs (including last months of life)
• Complex or escalating symptoms
• Life-threatening or catastrophic conditions

Eligibility

Patients must:

• Be aged 18+ (16–18 in exceptional circumstances with clinical agreement)
• Live in Mid Essex or be registered with a Mid Essex GP
• Give consent for referral

Out of area referrals may be considered in specific circumstances.

After referral

All referrals are reviewed and triaged by our clinical team.

We will contact the referrer or patient following assessment.

Please note: we do not provide long-term care beds. Inpatient Unit admissions follow a separate process.
