Accessing your personal information
Under the UK General Data Protection Regulation (UK GDPR), you have the right to request a copy of the personal information that Farleigh Hospice holds about you. This is known as a Data Subject Access Request (DSAR).
A DSAR may include information such as correspondence, records of interactions with the hospice, fundraising and supporter information, volunteer records, employment records (where applicable), and other personal information we hold about you.
To help us locate the correct information and protect your privacy, please provide your full name, date of birth and any other relevant details when making your request. We may need to verify your identity before releasing information.
Requesting healthcare records
If you have received care from Farleigh Hospice, you may request a copy of your health and care records. These records contain information relating to the care and treatment provided by the hospice, including assessments, care plans, clinical notes and correspondence relevant to your care.
Requests for health records can be made by the patient themselves, or by an authorised representative where permitted by law. We may ask for additional information to confirm identity and eligibility before releasing records.
We will respond to requests in accordance with applicable data protection and health records legislation.
