Care and support in your own home

Most people living with a life-limiting illness want to stay in the comfort of their own home. That’s why the majority of our care is provided in the community, whether in people’s homes or in care homes, so that you can remain in familiar surroundings, supported and at ease. Our community care teams work closely with you and your loved ones, tailoring support to your needs, preferences, and wishes. 

At any one time, we support over 500 people across our local community, providing compassionate care to those who need it.

What to expect from care at home 

When you receive care from our team, we focus on what matters most to you. Your care may include: 

  • Assessing and managing your symptoms and wellbeing 
  • Supporting your physical and emotional needs 
  • Practical guidance for day-to-day living 
  • Helping your family and carers access the support they need 
  • Short-term respite support for family or carers 
  • Guidance on accessing funded care and other local services 

Our community team includes Clinical Nurse Specialists, Registered nurses, therapists, and healthcare assistants with advanced training and expertise in Palliative and End of Life Care. Working collaboratively, the team coordinates and delivers high-quality, personalised support in patients’ own homes, ensuring continuity of care and responsiveness to individual needs.

You may also be supported at home by a specialist palliative care doctor, a member of our spiritual care or wellbeing team. 

How we work with other healthcare services 

We are a specialist service, working closely with your GP, hospital team and other NHS services to coordinate your care.  

We do not replace these services but provide guidance to help them deliver the care you need. Sometimes we may recommend that other healthcare professionals visit you, following our advice.
