Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region. Our team at Farleigh has grown and evolved to meet the changing needs of the community we serve, and we are looking for highly motivated Manager to join our Hospice Rapid Access Service (HRAs) team.
The Hospice Rapid Access Service (HRAs) is a collaborative partnership with other hospices in Essex which is commissioned by Essex Integrated Care Board.
The team facilitate rapid discharge from hospital or provision of care in the community for people with a primary health condition who are rapidly deteriorating, and within the last weeks of their life. This plays a critical role in ensuring people approaching the end of life receive compassionate, coordinated care in their preferred setting.
This involves a clinical assessment and regular review of care, with brokerage for provision of personal care at home, Nursing Home, or Inpatient stay at the Hospice.
This is an exciting opportunity for a senior leader to manage the fully established team of clinicians and care coordinators. This is a demanding and fast paced role, which would suit someone who can work at pace, manage competing priorities, and solve complex scenarios. There is a high degree of collaborative working required with our colleagues in the community, the acute trust, the ICB and Adult Social Care.
This role reports directly to the Director of Care and is responsible for oversight of all processes and performance of the Hospice Rapid Access team and direct line management of the staff in the team.
