Referrals for Psychological and Wellbeing Support

Our Psychological and Wellbeing Team support people living with life-limiting illnesses, and their families and carers, ensuring their emotional, social, spiritual and psychological needs are at the centre of their care. We offer bereavement support to those who are bereaved of a person who died under the care of Farleigh Hospice.

A guide to referrals

A referral can be made to Farleigh Hospice by health and social care professionals, people living with life-limiting illnesses, or their families or carers.

If your primary needs, or those of the person you are referring, relate to symptom management, additional support at home, advance care planning or end-of-life care, please refer to Farleigh Hospice via the clinical referral form (add link).

Anyone over the age of 18 who has a life-limiting illness, and anyone who is the family member or carer of a person with a life-limiting illness can refer for support or any person who is bereaved of a person who died under the care of Farleigh Hospice.

This service may be right if support is required with:

  • the emotional and psychological impact of living with a life-limiting illness
  • adjusting to changes in health, relationships or daily life
  • anxiety, worry, low mood or emotional distress
  • uncertainty about the future
  • practical or financial guidance
  • emotional and psychological impact of supporting a family member or loved one with a life-limiting illness
  • finding meaning, connection or comfort during times of uncertainty
  • enhancing wellbeing, quality of life and day-to-day living
  • finding ways to manage the emotional challenges of illness and everyday life
  • exploring thoughts, feelings and experiences in a safe, confidential and supportive space
  • understanding what support may be available
  • understanding grief following a bereavement

Referrals can be made by:

  • A person living with a life-limiting illness
  • Families and carers of a person living with a life-limiting illness
  • A person bereaved of someone cared for by Farleigh Hospice
  • Health and social care professionals

Farleigh Hospice offers a wide range of psychological and wellbeing support which may take place in a person’s home, in the community or at the hospice in Broomfield.

The team most suited to the person’s needs will lead on their care.

Our support may be offered as part of a group and/or one-to-one (in person or virtually)

Support may include:

  • Psychological, emotional, and spiritual support
  • Social and welfare support
  • Educational sessions
  • Peer support groups and activities
  • Bereavement support
  • Complementary therapy
Once a referral is received, our referral team will review the information to decide how we can best support the person. We may contact the person with the life-limiting condition, family/carer, or referrer for more details.

In consultation with the person being referred, we will then agree the most suitable support.

We work closely with our Farleigh Hospice colleagues, GPs, district nurses, hospital teams and other professionals to make sure care is coordinated and centred on the individual needs.
