It provides a safe space to ask questions, explore your options, and feel more confident in making decisions about your care.

• Managing medications safely and effectively at home
• Knowing who to contact with questions or concerns
• What advance care planning (ACP) means
• Exploring your wishes, preferences and priorities for future care
• How to start conversations about planning ahead

Who it’s for

Anyone living with a life-limiting illness, along with carers, family members and loved ones.

You can attend together or separately.
