Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region. Our team at Farleigh has grown and evolved to meet the changing needs of the community we serve, and we are looking for highly motivated Wellbeing Practitioner to join our Wellbeing Service team.
Based in Mid Essex, the successful candidate will join the extensive MDT and will undertake holistic assessments for patients and family members, working with them to provide practical and emotional support. This essential support may be delivered face to face in people’s homes or the hospice, in groups or virtually.
You will be a caring, compassionate and empathetic individual with proven experience of supporting people experiencing distress. You will have experience of working in health or social care and have excellent communication skills. You will have experience of advocacy and carer needs. You will have the ability to work autonomously as well as part of a multidisciplinary team. You will be a car driver with access to your own vehicle.
