Care in your own home
If you are living with a life-limiting illness you may prefer to be cared for in your own home.
Our Locality Care Team will work alongside your community nurse, GP and other health professionals to help improve your level of comfort, well-being and avoid unwanted hospital admissions.
Our team consists of trained nurses, therapists and health care assistants with specialist end of life training and experience. The care and practical support they can offer includes:
- Additional nursing care and emotional support to you and your family in the last days of your life
- Additional support and care on your discharge from the hospice or hospital while other services, for example, social care, are organised
- Respite in your own home to enable your family or those who care for you to continue caring
- Support you and your family at times of crisis to prevent unwanted and unnecessary hospital admissions particularly in the last few days of your life
- Advice regarding end of life care issues
- Guidance with medicines
- Administration of medications to relieve symptom
- Assessment and referral on to other services
- Liaising with other health care professionals to coordinate your care
- They can also just listen and be there for you and your family
Our Locality Care Team provides short-term care (for up to three months), between 8am and 8pm, 7 days per week.
Our specialist doctors and nurses will also be able to visit you in your own home to help with more complex symptoms and symptom control.
Therapy at home
Our therapists will be able to provide support via telephone, virtual consultations or to visit you in your home. They can help you maintain your mobility, independence and assess your equipment needs, all designed to keep you as active as possible. They can also help you manage complex symptoms such as fatigue and anxiety.
Telephone advice
Through our advice line, you will be able to talk to one of our clinical nurse specialists who will be able to advise on symptoms, nursing care and other needs relating to a life-limiting illness.
