Referral - Adult Bereavement

Bereavement can bring a whirlwind of emotions, and each person's experience is unique. Our bereavement teams are here to provide information, support and guidance to help you navigate the impact of grief.

The service will accept referrals as follows:

You can self-refer to our adult bereavement service using our online referral below.

We accept referrals based on the following criteria:

• You must be aged over 18 and be resident within mid-Essex and/or registered with a GP in the mid-Essex area covered by Farleigh Hospice
• You have experienced a significant bereavement which is directly impacting your ability to manage daily life
• At least 12 weeks have elapsed since the death

The service is unable to accept referrals from those who require emergency or crisis support.
