Rapid Discharge Team

Farleigh’s Rapid Discharge Service is designed to support quick discharge from Broomfield Hospital for those with a life-limiting illness requiring end of life care, where their preferred place of care is at home or Farleigh Hospice.

Our Rapid Discharge staff work with teams from across Broomfield Hospital to identify patients that are in their last few months of life and would like to return home, but need health and social care support.

Our nurses visit patients in the hospital and in partnership with a multidisciplinary team will identify individual care needs and collate a care plan, to enable the patient to return home or to their preferred place of care, as quickly as possible.
